HEREBY apply to have the Original 66 foot Lakeshore Road Allowance, or any portion thereof, on Lake Bernard in front of

the lands owned by me/us referred to above be stopped up, closed and conveyed to me/us and agree to the following terms and

conditions:

a) to obtain at our own expense, a reference plan of Survey satisfactory to the Townships solicitor of the Original

Lakeshore Road Allowance or portion thereof to be closed.

b) To pay all costs of Administration and advertising incurred by the Township in stopping up and closing to me/us

of the Original Lakeshore Road allowance

c) To pay all legal fees to convey to me/us of the Original Lakeshore Road Allowance

d) To pay to the Corporation of the Township of Strong as a purchase price for the Original Lakeshore Road

Allowance or Portion thereof a sum equivalent to $ .25 per square foot of the Original Lakeshore Road

Allowance.

e) To Accept title to the Original Lakeshore Road Allowance or portion thereof which is subject to such covenants

and conditions as may be required by the Corporation of the Township of Strong including such water easements

as may be reasonably required by the Corporation of the Township of Strong.

A non-refundable Fee of $300 is enclosed with this application to be applied toward the total expenses of Administration;

Advertising and the Land Costs.
